Description: This recall involves 2010-2012 Cannondale Bad Boy and Bad Girl commuter bicycles. They were sold in black. Only bicycles with date codes beginning with P, Q and RB through RL are included in this recall. The date code is located on the bottom of the bicycle fork. The frame is matte black. A Cannondale decal can be found on the downtube.

 

Incidents/Injuries: The firm has received 30 reports of the bicycle’s fork axle cracking; including an injury report of a fall that resulted in a concussion and bruising.

Sold at: Authorized Cannondale dealers nationwide from June 2010 through December 2014 for between $1,000 and $1,800.

Name of Product: Rope cocking devices for Centerpoint Sniper 370 Crossbows

Hazard: Tabs on the hooks attaching the rope cocking device to the crossbow string can bind as the device is pulled back, causing the hooks to break and the rope to recoil, posing a risk of injury, including laceration.

 

Remedy: Replace

Consumers should immediately stop using the recalled rope cocking device and contact Crosman to receive instructions on returning it for a replacement rope cocking device without tabs. Crosman has contacted all known purchasers.

Description: This recall involves rope cocking devices shipped by Crosman Corporation with Centerpoint Sniper 370 crossbows with serial numbers beginning with “015.” The serial number is printed on the right side of the crossbow above the trigger. The Centerpoint Sniper 370 crossbow has a camouflaged stock and measures 36.5 inches long. The rope cocking device is a rope with a black plastic “T” shaped handle at each end. A pair of black plastic tabbed hooks are connected to the rope. The rope cocking device measures 45 inches from handle to handle. The words “CENTERPOINT” and “SNIPER 370” are printed on the crossbow’s frame. 

 

Incidents/Injuries: Crosman has received three reports of broken hooks on the rope cocking device, including one report of a facial laceration.

 

Sold at: Academy, Cabela’s and other stores nationwide and online at Amazon.com, Crosman.com and Sportsmansguide.com from December 2015 through June 2016 for about $350.

Description: This recall involves Lorex Care ‘N’ Share video baby monitors. The model numbers included in this recall are WL3520, WL4320 and WL3401. The model numbers are printed on the back panel of the monitor. The monitors were sold in bundles with cameras. The monitors contain a blue lithium polymer battery and measure about 4 inches tall by 5 inches wide. The monitors have a white plastic back and either a white or black border. “LOREX” or “The Lorex Baby” is printed below the monitor screen.

 

Incidents/Injuries: The firm has received 488 reports of batteries overheating and expanding; about 140 reports involved the swelling of the battery pack, causing its plastic casing to open or come off.  No injuries have been reported.           

Sold at: Babies “R” Us, B&H, Best Buy, Target, The Home Depot, Walmart and online at www.amazon.com and www.lorextechnology.com from May 2013 through April 2016.  The video baby monitors were sold in bundles for between $60 and $140.

CDC awards $60 million to help states and territories battle Zika

Funding is stopgap measure to help states respond to emerging threat, protect pregnant women

 

The Centers for Disease Control and Prevention (CDC) will begin making awards totaling nearly $60 million to states, cities, and territories to support efforts to protect Americans from Zika virus disease and adverse health outcomes that can result from Zika infection, including the serious birth defect microcephaly.

 

The funding, distributed through CDC's Epidemiology and Laboratory Capacity for Infectious Diseases Cooperative Agreement (ELC), will support activities to protect the health of the American public, especially pregnant women, including epidemiologic surveillance and investigation, improving mosquito control and monitoring, and strengthening laboratory capacity. It will also support participation in the U.S. Zika Pregnancy Registry to monitor pregnant women with Zika and their infants, as well as Zika-related activities in U.S.-Mexico border states.

 

"Local, state and territorial health departments are on the front lines in the fight against Zika," said CDC Director Tom Frieden, M.D., M.P.H. "These CDC funds will strengthen state and territorial capacity to respond to Zika virus, an increasingly concerning public health threat for pregnant women and babies. We hope Congress will provide the additional resources we need to fully support the Zika response."

This new funding will be available to jurisdictions August 1 and is in addition to $25 million awarded on July 1 as part of CDC's preparedness and response funding to states, cities, and territories in areas at risk for outbreaks of Zika. On August 1, CDC also will award another $10 million to states and territories to quickly identify cases of microcephaly and other birth defects linked to Zika and to refer affected families to services.

 

The Obama Administration has requested $1.9 billion that public health experts identified as necessary to combat Zika and protect the homeland, but the budget has not been approved by Congress. While this ELC funding is an important tool to help communities prepare for and respond to infectious diseases like Zika virus, additional support will be needed to help further expand mosquito control capabilities and develop a Zika vaccine and diagnostics, among other priorities.

 

Zika virus spreads to people primarily through the bite of an infected Aedes species mosquito (Aedes aegypti and Aedes albopictus), although Aedes aegypti are more likely to spread Zika. Zika infection can also be spread by men and women to their sex partners. There is currently no vaccine or treatment for Zika. Zika infection is often asymptomatic; among those who have symptoms, the most common complaints are fever, rash, joint pain, and conjunctivitis (red eyes). The illness is usually mild with symptoms lasting for several days to a week after being bitten by an infected mosquito. However, Zika infection during pregnancy may cause microcephaly and other severe brain defects in the developing fetus. Zika also has been linked to Guillain-Barré syndrome, an uncommon sickness of the nervous system in which a person's immune system damages nerve cells, causing muscle weakness and sometimes paralysis or death.

 

Background

 

CDC's Epidemiology and Laboratory Capacity for Infectious Diseases Cooperative Agreement (ELC) distributes funding annually to support an array of federal projects to strengthen the ability of domestic public health departments to respond to emerging and re-emerging infectious disease threats. This year, which includes increased funding for Zika and fighting antibiotic resistance, ELC has awarded $240 million to help states detect, prevent, and respond to the growing threats posed by infectious diseases, including foodborne and vaccine-preventable diseases. Last year's ELC award was nearly $110 million.

Name of Product: Flushmate® III Pressure-Assist Flushing Systems

 

Hazard: The system can burst at or near the vessel weld seam releasing stored pressure. This pressure can lift the tank lid and shatter the tank, posing impact and laceration hazards to consumers and property damage.

Remedy: Repair

 

Consumers should immediately stop using the recalled Flushmate III systems, turn off the water supply to the units, flush the toilet to release the internal pressure and contact the firm to request a free repair kit.

Units: About 453,000 in the U.S. (in addition, about 12,000 in Canada) (The Series 503 Flushmate III was previously recalled January 2014 and June 2012.)

 

Description: This recall is for Series 503 Flushmate III Pressure Assist flushing systems installed inside toilet tanks that were manufactured from July 2009 through April 2011. The units are rectangular, black, two-piece vessels made of injection molded plastic. Previously recalled systems were manufactured from October 1997 through June 2009. Recalled units have a date code/serial number that is 16 characters long and is located on the label on the top of the Flushmate III. The first six numerals of the serial number are the date code. The date code range for units included in this recall in MMDDYY format is 070109 (July 1, 2009) through 043011 (April 30, 2011).  The date code range for previously recalled systems in MMDDYY format was 101497 (October 14, 1997) through 063009 (June 30, 2009). Units included in this recall were sold individually and installed in toilets manufactured by American Standard, Copperfit, Crane, Ecotech, Eljer, Gerber, Kohler, Lamosa, Mancesa, Mansfield, Orion, St. Thomas, Universal Rundle, Vitra, Vitromex, Vortens and Western Pottery.

 

Incidents/Injuries: Flushmate has received two reports of the units included in this recall bursting, resulting in property damage, including one report of a minor injury.

 

Sold at: Home Depot and Lowe’s stores, toilet manufacturers, distributors and plumbing contractors nationwide from 2009 through 2011 for about $108 for the units without toilets.

Description: This recall involves brown metal floor lamps with alabaster glass shades. They are illuminated with a single 100-watt light bulb and measure about 70 to 72 inches tall. The model numbers LMP4229, LMP4168 and LMP10771 can be found on the label at the bottom of the lamps. 

 

Incidents/Injuries: Home Source has received three reports of the lamp’s sockets burning and smoking.  No injuries have been reported.

Sold at:  Small retail stores located in the New York metropolitan area and online at Amazon.com, Houzz.com, Target.com and Wayfair.com from February 2016 to April 2016 for about $60.

Name of Product: Liquid Plumr® Pro-Strength Foaming Clog Fighter®, Liquid Plumr® Industrial Strength Urgent Clear® and Liquid Plumr® Pro-Strength Urgent Clear® clog removers

 

Hazard: The recalled bottles are not child-resistant and children can remove the top, posing a risk of chemical burns and irritation to the skin and eyes. These products contain sodium hydroxide which is required by the Poison Prevention Packaging Act to be sealed with child-resistant packaging.

 

Remedy: Refund

Consumers should keep this recalled product away from children and immediately contact The Clorox Company’s Liquid Plumr call center for disposal and refund instructions.

Units: About 80,000 (in addition, about 28,000 were sold in Canada). The PATRULL KLÄMMA safety gates were previously recalled in May 2015.

 

Description: This recall involves IKEA PATRULL, PATRULL FAST and PATRULL KLÄMMA safety gates and safety gate extensions. The gates and gate extensions are white, made of beech wood or steel and plastic. The pressure-mounted gates open both inward and outward. They measure about 28 ¾ inches high with an adjustable width from about 28 ¾ inches to 34 ¼ inches. The gate extensions measure about 28 ¾ inches high with an adjustable width from about 2 ½ inches to 5 inches.

 

A permanent label is attached to the metal bar at the bottom of the safety gate containing “IKEA” and the article number. Gates with the following article numbers are included in this recall:
