Advanced Pharma, Inc. d/b/a Avella of Houston is conducting a voluntary recall of all unexpired sterile injectable products labeled "latex free" that were produced at Advanced Pharma, Inc.'s Houston location between September 1, 2016 and February 16, 2017 to the user level (hospitals and institutions) because such products may contain synthetic latex and/or natural latex. Avella and Advanced Pharma have been unable to confirm with clarity whether its "latex free" label statements are accurate in all cases and are therefore conducting this recall in an abundance of caution.

Recall Summary

 

Name of Product: Contemporary Cutlery knives

 

Hazard: The blade on Contemporary Cutlery knives can break during use, posing a laceration hazard.

 

Remedy: Replace

Consumers should immediately stop using the recalled cutlery and contact Calphalon for a replacement cutlery product.

Recall Details

 

Units: About 2 million (in addition, about 7,000 were sold in Canada)

 

Description: This recall involves Calphalon Contemporary Cutlery carving, chef, paring, santoku and utility knives sold individually and in sets made between August 2008 and March 2016. The following models are included in the recall available here:

 

Incidents/Injuries: Calphalon has received 27 reports of finger or hand lacerations; including four injuries requiring stitches. In addition, the firm has received about 3,150 reports of broken knives.

 

Sold at: J.C. Penney, Kohl’s, Macy’s and other stores nationwide and online at www.Amazon.com from September 2008 through December 2016.  The recalled knives were sold from September 2008 through December 2016 for $25 for a single knife to $300 for a knife block set.

Moose Toys Recalls Toy Frogs Due to Chemical and Injury Hazards

 

Recall Summary

 

Name of Product: Little Live Pets Lil Frog and Lil Frog Lily Pad toys

 

Hazard: When the button batteries are removed from the toy frogs, the battery’s cap can become a projectile and the battery’s chemicals can leak, posing chemical and injury hazards.

 

Remedy: Replace

Consumers should immediately stop using the toy frogs, refrain from opening the battery compartment and contact Moose Toys for a free replacement Little Live Pet product.

Recall Details

 

Units: About 427,000 (in addition, 17,800 were sold in Canada)

 

Description: This recall involves the Little Live Pets Lil Frog plastic toys. They operate with four button batteries and jump. Little Live Pets Lil Frog has SKU: 28217 and Lil Frog Lily Pad has SKU: 28218 printed on the frog’s lower belly near its left thigh with a manufacture date code under it. The date code range is WS112016 to WS123216. The toy frogs were sold in pink, blue and green colors.

 

Incidents/Injuries: The firm has received 17 reports of the battery’s cap becoming a projectile or battery chemicals leaking, including two injuries that resulted in emergency room and doctor’s office visits for eye irritation from the battery chemicals.

 

Sold at: AAFES, Target, Toys “R” Us and Walmart stores nationwide and online at Amazon.com from August 2016 through February 2017 for about $15 for the Lil Frog and $25 for the Lil Frog Lily Pad.

Organic Herbal Supply, Inc. announced today that it is conducting a voluntary nationwide recall of all lots of XtraHRD Natural Male Enhancement capsules. FDA analysis has found the product to contain Tadalafil. Tadalafil is a FDA-approved drug used as treatment for male Erectile Dysfunction (ED). The active drug ingredients are not listed on the label for this product. To date, no adverse events have been reported.

Deutsch Kase Haus, LLC of Middlebury, Indiana, supplied MDS Foods with cheeses that have been found to be contaminated with the pathogenic organism Listeria Monocytogenes which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women. At the time of this release, no illnesses have been reported.
