(NYSE: HSP), announced today it is initiating a voluntary user-level recall of one lot of Lactated Ringer’s and 5% Dextrose Injection, USP, 1000 mL, Flexible Container, NDC 0409-7929-09. This action is due to one confirmed customer report where a leak was identified in the primary container between the cobra cap and fill-tube seal and a spore-like structured particulate, consistent with mold, was noted in the solution.

Whole Foods Market announces that it is recalling Kenny’s Farmhouse Cheese KY BL sold in its Louisville, KY store and Blue Kentucky Rose Kenny’s cheese sold in its Nashville, TN store. These cheeses are being recalled because they have the potential to be contaminated with Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems.

The Protein Bistro Box contains peanut butter with one of the lot codes recently added to the Justin’s recall which impacts peanut butter packs with Best By dates of 8/26/13.

KRASNYI OKTYABR located at 60 20th street, Brooklyn, NY 11232 is recalling Beer Set Dried Rudd discovered by New York State Department of Agriculture and Markets Food Inspectors during a routine inspection and subsequent analysis of the product by Food Laboratory personnel that confirmed the fish had not been properly eviscerated prior to processing. This product may be contaminated with Clostridium botulinum spores, which can cause Botulism, a serious and potentially fatal food-borne illness.

FDA is working closely with the CDC and state partners to investigate an outbreak of meningitis among patients who had received an epidural steroid injection. Investigation into the exact source of the outbreak is still ongoing, but the outbreak is associated with a potentially contaminated medication.

UPDATE: Multistate Meningitis Outbreak Investigation
CDC is coordinating a multistate investigation of meningitis among patients who received epidural steroid injections (medication injected into the spine). Several of these patients have had strokes related to the meningitis. In several patients, the meningitis was found to be caused by a fungus that is common in the environment but rarely causes meningitis. This form of meningitis is not contagious. The source of the fungus has not yet been identified, and the cause of infections in the other patients is still being assessed.

Health Advisory – Multi-state Meningitis Outbreak

Health Advisory

The Centers for Disease Control (CDC) is coordinating a multistate investigation of meningitis among patients who received a steroid injection (medication injected into the spine or other joints). Some medications used in these treatments have been linked to 35 cases of fungal meningitis, including five deaths, across the United States.
